NEW STUDENTS (PK only)
- Step 1: Students who are entering PK for the Fall of 2020, please click here to complete online registration.
- Step 2: Complete additional enrollment forms listed here:
- PK forms
- JOM (if your child has a CDIB card
- School Lunch Application
- Email to Lindsay Elementary (LES@lindsay.k12.ok.us). Please put your child's name as the subject line on the email.
- Step 3: Along with the enrollment forms listed in Step 2, submit a copy of your child's:
- Birth certificate
- Immunization record
- Proof of residency (electric bill, water bill, rent/lease contract).
- Copies can be emailed to Lindsay Elementary (LES@lindsay.k12.ok.us).
- Step 4: Complete the following bus transportation request - Bus Transportation - if your child is planning to ride the bus.
If you are unable to send forms electronically, please print forms and drop them off at the LES Enrollment Event which is scheduled for Wednesday, May 20th.
NEW STUDENTS (KG - 5TH)
- Step 1: Students who are entering KG through 5th grade at Lindsay Elementary for the first time need to complete the following forms.
- New Student Form
- JOM (if your child has a CDIB card)
- School Lunch Application
